ElderGrace is a seasonal prayer and reflection resource complied by Most Blessed Sacrament Parish. Each ElderGrace booklet is mailed to parishioners over 75. If you or someone you know would like to receive the ElderGrace booklet in the mail, please fill out the 65+ Annual Update form on this page or contact the parish office at (920) 231-9782.
Most Blessed Sacrament provides communion to those parishioners that can no longer attend Mass due to physical restrictions. A Eucharistic Minister will bring communion to your home or to a nursing home facility upon request. Please email Laurie or call (920) 231-9782 to leave your name and address if you wish to receive communion at home.
If you or a family member is in need of a listening presence, additional support or spiritual companionship we can help to provide that comforting measure. Please email Laurie or call (920) 231-9782.
Most Blessed Sacrament Care Team Ministry provides compassionate care and assistance to both our parish family and our local community. Team members are trained to provide respectful, non-medical assistance and are led by Care Team leaders with support from the Pastoral Staff who have been trained through Catholic Charities.

Care Ministers are trained through a program provided by the Diocese of Green Bay. The responsibilities of a Care Minister include:
Bearing witness to God’s love for all
Responding to the spiritual and emotional needs of the bereaved, hospitalized, terminally ill, separated, divorced, unemployed, relocated, and others facing a crisis or life challenge
